Living Well Medina County named Chronic Disease Prevention one of the top priority areas to address in the 2024 – 2030 Community Health Improvement Plan. Many organizations, teams, coalitions, and agencies are working hard to implement evidence-based strategies. Below you will see our goals, strategies, and assessments that will help us reach our 2030 goals in Medina County.
Goals

- Improve percentage of residents who received a routine check-up in the past year from 72.8 % to HP2030 target of 84%
- Improve screening for breast cancer from 73.5% to HP2030 goal of 80.3%

- Reduce food insecurity from 8.3% to HP2030 goal of 6%.

- Increase the percentage of mothers who receive prenatal care from current 73.2% to HP2030 goal of 80.5%
- Increase from 86.7% to 90% required immunizations completed among children entering kindergarten.
