Creating a Group

Any group, coalition, or project that is eager to improving Medina County can launch an action group. If you’d like to start a new action group, contact engage@medinahealth.org or click here to get started.

Customizing Your Profile

Once logged in, click your profile or the arrow dropdown on the website menu. You can click profile or settings. This will redirect you to all the tabs that allow you to customize your profile.

The Profile Tab allows you to edit:

  • Your Name
  • Whether you work in Medina
  • Who can see the above field
  • Profile Photo
  • Cover Image

The Settings Tab allows you to edit:

  • Your Email
  • Your Password
  • When to send an email notice
  • Profile Visibility

How to Use Action Groups

To join a group, click the “Groups” tab under the Community Planning Tool link on the website menu. Once you find a group you would like to join, simply click the “Join Group” button.

Each group will show a brief description, the amount of members, when it was last active, and the visibility of the group

Every group will have tabs that hold all the important information regarding the group. It helps us organize information in sections so different types of information can be accessed easily.

The “Home Tab” is an activity feed that displays everything that has happened in that group. It shows important updates and messages, and is a great way for new members to see what’s going on.  If you have anything to share or ask, you can always post an activity message.

The “Strategy” tab shows the overall strategy of the group. It has  many sections that help us outline our strategy in hopes of achieving our goal.

The “Raw Data +” tab shows all the data for the action group. We use the Community Health Assessment and Clear Impact to update all our data and information.

The “Performance and QI” tab shows all the charts, graphs, and visuals that are displayed based on the data that was entered in the Raw Data+ tab. They also implement the Plan Do Study and Act (PDSA) process to track strategies that are being implemented

The “CHIP Dash” tab displays the groups implementation with the Community Health Improvement Plan (CHIP)

The “Media” tab holds all the media like posters, flyers, graphics, handbooks, and any resources that are published in the group.

The “Members” tab displays all the members that joined that specific group.

The “Funding” tab displays all the group funding/grants, including any details regarding that grant
